We know that it is important for each patient to understand their health insurance coverage, and we are happy to verify your coverage for you before your first visit. Simply fill out the form below.

Some of the major medical insurance plans we are in-network with include: 

*Certain services like OFFICE VISITS may be covered when provided by our chiropractic physicians.

** Under certain circumstances massage therapy may be covered when you have an examination by a chiropractic physician first.

FUNCTIONAL MEDICINE & HEALTH COACHING

Specialty labwork and Supplements: Patients may be recommended specialty lab work and supplements. Many of these labs and supplements are not covered by Insurance but may be eligible for payment by health savings or flexible spending accounts.

If You Don’t See Your Health Insurance Plan Listed Above

Many independent insurance companies have partnerships that include you in the network of the larger insurance companies or provide in-network benefits. We can check your coverage for you when you fill out the form below. 

What if you have Insurance, but we are not in-network with your insurance company?

That’s OK. We help lots of patients get the care they need. We provide a receipt with everything required to submit the claim to your insurance company so they can reimburse you directly. You can still fill out the form below, and our team can verify your benefits. We will review them before your first visit and explain your coverage.
