Scheduling Your New Patient Appointment
We layer services for our patients to receive as part of their treatment plan so they can get the most effective treatment at the right time of their recovery. Most services are also offered as stand-alone treatment options. These services can be broken down into categories: Chiropractic Care, Shockwave Therapy, Class IV Laser Therapy, Physical Therapy, Massage Therapy, Custom Orthotics, and Functional Medicine.
Our Chiropractic Services are delivered by a team of experts who are dedicated to getting to the root of your pain. We are a family practice with a no-nonsense approach to your care, serving patients of all ages with various conditions and injuries. Our team is certified in the patented Webster technique, which is particularly beneficial for expectant mothers, as well as Selective Functional Movement Assessments and AIMLA certified (American Institute of Medical Laser Application), ensuring that you receive the highest quality of care.
Shockwave Therapy is a revolutionary, non-invasive treatment that harnesses the power of acoustic waves to kickstart your body’s natural healing process. It’s a game-changer for chronic musculoskeletal pain, providing relief in areas like the shoulder, elbow, knee, and heel. By boosting blood flow and encouraging the growth of new blood vessels, Shockwave Therapy can speed up tissue repair and regeneration, leading to less pain, better mobility, and quicker recovery from injuries. Pregnancy Care: Our wellness center specializes in providing expert physical medicine for pregnancy-related issues through our Webster-certified chiropractic care and certified prenatal massage therapy. Our team understands the unique needs of expectant mothers and focuses on offering safe and effective treatment options to support their well-being during this particular time. Our Webster-certified chiropractic care is tailored to address the specific musculoskeletal changes and discomfort often accompanying pregnancy, promoting proper alignment and relieving tension. In addition, our prenatal massage therapy is also performed by licensed professionals trained to address the unique needs of pregnant women, providing gentle and soothing massage techniques to alleviate discomfort and promote relaxation. With our comprehensive approach to prenatal care, we aim to support the health and comfort of expectant mothers as they prepare for the arrival of their little ones.
Physical Therapy: Our physical therapy program utilizes a comprehensive approach that focuses on creating a stretching and exercise routine you can perform at home that will help restore your strength, flexibility, and stability.
Massage: Ravenswood Chiropractic & Wellness Center’s licensed massage therapists specialize in seven types of massage therapy, including prenatal and sports massage. They can help you achieve physical wellness.
Functional Medicine: We are what we eat, and an approach to wellness that includes a focus on healthy eating habits and physical fitness can have numerous benefits. We will help you get there by supporting a healthy diet based on whole foods, regular checkups, and natural interventions. We also offer comprehensive testing, including food allergy/sensitivity, digestive, and hormone testing.
Our patients can schedule or reschedule appointments in-person, online patient portal, by text, or over the phone just like they could in any medical office.

If you have been seen in our office before you should have received a personal link to your online scheduling portal, if you need assistance or require a new link please let us know by calling us or texting us at 773.878.7330 or submitting your request here.
We understand circumstances can change, and we kindly request patients to notify us at least 48 hours in advance if they need to cancel or reschedule an appointment. This not only helps us manage our schedule effectively but also allows us to offer the time slot to another patient in need, thereby benefiting the entire patient community. If, however, you cancel your appointment within 48 hours and we are unable to fill your slot, a cancellation fee will be assessed to cover the full cost of the appointment.

We are physical medicine specialists who diagnose and treat bone, muscle, ligament, tendon injuries using a range of traditional and advanced technologies, such as Class IV Laser and Shockwave Therapies.

You will be asked to fill out New Patient Forms which will help us learn about you and your condition. Please take your time to fill them out completely. The more information you are able to provide, the better we will be able to help you. You will be sent the forms to fill out digitally once you schedule your first appointment, or you can wait to fill them out once you’re here. All forms will remain strictly confidential (see our privacy policy for more information).
The doctor will ask you questions to assess your condition and how it is affecting you, and then will proceed with an examination. Nothing will be done in our office without your consent.
You will then receive a thorough physical exam which will include standard orthopedic, neurological, postural, and other examinations as needed. After the initial Examination, the doctor may order additional testing to be performed. The doctor will do this to determine the cause of your condition. No additional testing will be performed in our office without a full explanation of the procedure, your options, and your consent.
Additional Testing which may be ordered may include: X-ray, CT, MRI, Ultrasound, EKG, Stress Test, or Laboratory Testing. If you have recently had testing done, please bring in copies of the films, reports, and laboratory results to your first appointment along with a list of your current medications (include herbal and over the counter).
After the doctor has gathered and analyzed your information, examination, imaging, and testing results, the doctor will review his findings with you and explain recommendations for treatment.
Hand Washing and Hand Sanitizer: Use of the sinks located in the bathrooms or hallway outside the Physical Therapy Room is encouraged. Hand Sanitizer is available for your use in the reception area.
Cleaning: Our team cleans treatment rooms with medical-grade disinfectant after each patient visit. Common touch points and surfaces throughout our office are cleaned hourly with the same medical-grade disinfectant. Extensive cleaning of the office is done at the end of each day.

After your Chiropractic Exam, you and your doctor will form a treatment plan personalized to your needs. No two patent’s treatment plans are exactly alike, so this is a collaborative process between you and your doctor in which they state what you need in order to get well, and you state what you can commit.
Once your treatment plan is established, you’ll receive your treatments, and the doctor will continue to evaluate your condition and track your progress until you get better.
Paying for Your Care
We accept most insurance plans, as long as it’s a PPO. Listed below are some of the providers that we accept.

We accept insurance plans from many providers that are not listed above. If you have a PPO insurance plan that is not listed above, if you have questions about your policy, or would like your insurance verified before your first visit, you can submit your insurance information here and we will verify your available coverage in our office and our patient services coordinator will review it with you. Unfortunately, we do not accept any HMO, Medicaid, or AllKids insurance plans.
If you do not have insurance or our services are not covered by your plan we offer ChiroHealth USA.
ChiroHealth USA is a discount membership program that allows us to give you a discount off our regular fees. It provides a minimum discount of 33%+ and provides a maximum fee for Chiropractic treatment with or without an examination. There is an annual fee of $49 paid directly to ChiroHealth USA which covers the purchaser and their spouse and or partners and dependents for one full year. The cost of the membership fee is usually saved in one visit. We receive no compensation from ChiroHealth USA.
